How to process our complaint form?
Please explain the complaint in as much detail as possible and answer the questions in the complaint form as complete as possible so that we can thoroughly check the information you provide to us. Upon receipt of the form, we will send you a confirmation of receipt.
For data protection reasons, please note that only information without reference to a patient or specific patient group should be submitted. Documents sent together with the complaint must be made unrecognizable with regard to the patient's name, patient abbreviations or patient codes. The data and information you provide will be treated with strict confidentiality and will only be used to process and analyze your complaint. We will not provide your data to any third parties, except our obligation to provide information to any reporting and monitoring authorities/notified bodies.
